Perineal tears and episiotomy often occur in childbirth with first-time deliveries, but the risk of these injuries can be reduced by preparing the perineum, often through massage.

Aside from uterine rupture risk, the drawbacks of VBAC are usually minor and identical to those of any vaginal delivery, including the risk of perineal tearing.

Many patients attribute their symptoms to an antecedent event (eg, post-partum faecal incontinence associated with a perineal tear) or they may present with chronic symptoms that have recently progressed (eg, stress urinary incontinence or chronic constipation).
