The diagnosis is based on the two constant features, a typical eruption and the characteristic histologic features; thus the eponym "Sweet's syndrome" is used.

Eruptions can occur frequently with a certain drug (for example, with phenytoin), or be very rare (for example, Sweet's syndrome following the administration of colony-stimulating factors).

Such reactions include Stevens-Johnson syndrome, Sweet's syndrome, toxic epidermal necrolysis (TEN), and painful and disfiguring rashes.
