The patriarch of Bulgaria was exiled here from his office in Veliko Trnovo, the old capital, when the Ottomans captured the country.

The caves in the region had been inhabited by monks from the 1220s, when it was founded by the future Patriarch of Bulgaria Joachim, to the 17th century, where they hewed cells, churches and chapels out of solid rock.

Built in Neo-Byzantine style, it serves as the cathedral church of the Patriarch of Bulgaria and is one of the largest Eastern Orthodox cathedrals in the world, as well as one of Sofia's symbols and primary tourist attractions.

It was only in 1285 that the Patriarch of Bulgaria Joakim III arrived in Constantinople and arranged for the release of Theodore Svetoslav, who was now supposed to marry a daughter of the high court official John Synadenos.

Stephen Uroš IV Dušan had himself and his wife crowned emperor and empress by patriarchs of Bulgaria and Serbia, as he was aware that the Patriarch of Constantinople wouldn't bestow the imperial title upon ruler of a Balkan country.

The square was named after Evtimiy of Tarnovo, Patriarch of Bulgaria from 1375 to 1393 and one of the most important figures of medieval Bulgaria; a monument to Evtimiy by sculptor Marko Markov has adorned the square since 1939.

These actions, which the Byzantines received with indignation, appear to have been supported by Bulgaria, as the Patriarch of Bulgaria Simeon had participated in both the creation of a Serbian patriarchate and the imperial coronation of Stefan Uroš IV Dušan.

In 1345 Stefan Uroš IV Dušan proclaimed himself Emperor (Tsar) and was crowned as such at Skopje on 16 April 1346 by the newly created Patriarch of Serbia, and by the Patriarch of Bulgaria and the autocephalous Archbishop of Ohrid.
